Why remediation evidence often falls short
After a complaint, a notification or conditions on registration, many practitioners do a great deal of learning and still struggle to show it. Certificates pile up without a clear link to the concern. Reflections describe courses rather than change. Supervisor reports arrive late and say little. The result can look like activity rather than remediation.
Good remediation evidence tells a coherent story: this was the concern; this is what I understood about it; this is what I did; this is what I learned; this is how my practice is now different; and this is independent evidence that the change is real and sustained. The Remediation Evidence Portfolio is built around that story, and organises your material around the concerns rather than around the activities.

How the builder works
Record the concern
Write a brief, factual, de-identified summary of the concern and where it came from: a notification, an employer, a complaint or your own review.
Diagnose the gaps
Break the concern into specific gaps, such as knowledge, skill, judgement, behaviour, health or systems, and link each to the relevant part of your code of conduct or Board standard.
Set objectives you can evidence
For each gap, write an objective describing a specific, observable change, not just a course to complete.
Plan activities and support
List the education, supervision, mentoring, audits or system changes for each objective, with providers, dates and who will support you.
Collect evidence as you go
Attach certificates, assessment results, before and after audits, new templates or protocols, and supervisor or mentor reports to the objective they support.
Reflect on learning and change
Write reflections linked to each objective, and a reflective report that draws the whole story together.
Review and export
Check every objective has a matching activity and evidence, then export a structured portfolio to review with your adviser.
Types of evidence and what each shows
| Evidence | What it shows |
|---|---|
| Completion certificates and assessment results | That you finished the education and met its standard |
| Reflective practice report | Your understanding of the concern, what you learned and how you have changed |
| Supervisor or mentor reports | An independent view of your practice and behaviour over time |
| Audit results before and after | Measurable change in records, prescribing or other practice |
| New protocols, templates or systems | Concrete changes to how you work |
| Feedback from colleagues or patients | How others experience your practice now |
What the tool includes
Everything you need to build the portfolio, and nothing that pretends to be legal advice.

Prompts to break a concern into specific gaps and connect each one to the code, standard or guideline it relates to.
Objective and activity planner
A plan template that keeps each objective, activity, support arrangement and evidence item linked.
Evidence library
Upload and label documents against objectives, with reminders of what each type of evidence can and cannot show.
Insight and reflection prompts
Prompts drawn from our courses on insight and reflection, covering what happened, why, its impact on others and what has changed.
Progress view
See at a glance which objectives still lack an activity, evidence or reflection.
Structured export
Download the portfolio as an organised document for your adviser, supervisor or your own records.
Important limits
- This is not legal advice. The tool helps you organise your own material. Whether, when and how anything is provided to Ahpra, a Board, a co-regulatory body, a panel or a tribunal is a decision to make with your indemnity insurer, medical defence organisation or lawyer.
- No portfolio can promise an outcome. Decision-makers weigh many factors, and the strength of evidence is only one of them.
- Check your requirements first. If you have conditions or undertakings, the education, educator, mentor or supervisor may need to be nominated and accepted before activities count. The portfolio records what you do; it does not replace those steps.
- Write in your own words. Decision-makers look for authentic insight. Prompts help you think, but the content must genuinely be yours.
- The tool and our courses are not accredited and not approved by Ahpra or any National Board.

What you get for A$79
Twelve months of access in your student portal, the full builder, evidence attachments and exports at any time. Remediation often takes months, so you can return to the portfolio as supervision reports and audits arrive, and add follow-up reflections showing that change has been sustained.
If you are at the very beginning, read our guide to Ahpra notifications first and get advice. If you want structured learning alongside the portfolio, the Insight, Reflection and Remediation Program groups the courses most closely linked to this tool.

Frequently asked questions
When should I start a remediation portfolio?
As early as is sensible, ideally after you have taken advice. Starting early lets you plan learning around the actual concern and collect evidence as you go, rather than reconstructing it later.
Will a Board accept my portfolio?
No one can promise that. A Board or other decision-maker considers the whole matter. A clear, honest portfolio helps you present your learning and change, but it cannot determine the outcome.
Do I need conditions to use this tool?
No. Practitioners also use it after an employer concern, an incident or a complaint, and before responding to a notification, to plan and evidence learning in a structured way.
What should a reflective report include?
Ahpra's information sheets describe a four-part structure based on Gibbs' cycle: description, feelings and response, evaluation and analysis, and conclusion and change. Doctors should check the Medical Board's requirements, as it has not adopted those sheets.
Can my lawyer or supervisor see the portfolio?
Only if you export it and share it. Your portfolio is private to your portal account.
What if my remediation is not working?
Record that honestly and speak to your supervisor and adviser. Adjusting the plan is better than presenting evidence that does not reflect your practice.
Does the tool cover NSW or Queensland matters?
The structure works for any process, but the bodies and terminology differ in New South Wales and Queensland. Get advice specific to the body handling your matter.
